About the role
Key responsibilities & impact- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide complete physical assessments and histories of current and previous illnesses by phone
- Provide professional nursing care using all elements of the nursing process
- Assess and evaluate patient status
- Use health assessment data to determine nursing diagnoses
- Develop care plans with goals based on nursing diagnoses and palliative nursing actions
- Include patients and families in the care planning process
- Initiate preventive and rehabilitative nursing procedures
- Administer medications and treatments as prescribed in the physician’s plan of care
- Counsel patients and families regarding nursing and related needs
- Provide health care instructions according to assessment and care plan
- Assist patients with activities of daily living and facilitate self-sufficiency and comfort care
- Act as case manager and coordinate patient care for assigned caseload
Requirements
What you’ll need- Must have a valid RN license in the state of California
- Minimum of two years of experience; hospice experience is required
- Must have Home Care Home Base experience
- Maintains a current CPR certification
- Demonstrates excellent observation, problem solving, verbal and written communications
- Nursing skills per competency checklist
- Ability to organize and prioritize workload independently
- Able to lift, position, and/or transfer patients
- Able to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- Self-directed and able to work with minimal supervision
- Prolonged or considerable walking or standing
- Considerable reaching, stooping, bending, kneeling, and/or crouching
