About the role
Key responsibilities & impact- Adhere to the care plan to ensure safe and competent patient care.
- Assist patients with personal hygiene and maintain a clean, safe, and healthy environment.
- Help patients with physician-prescribed ambulation under clinician or therapist approval or supervision.
- Support therapy personnel during rehabilitative processes.
- Encourage maximum patient independence according to the nursing care plan.
- Promote mental alertness through activities of patient interest.
- Provide emotional and psychological support to patients and household members while fostering trust and confidentiality.
- Report changes in the patient’s condition or home environment to the RN or clinical manager.
- Perform light housekeeping tasks as directed by the clinician.
- Complete visit reports promptly and accurately in the electronic medical record system.
- Confirm daily visit schedules for coordination with other staff.
- Participate in the agency’s Quality Assurance and Performance Improvement (QAPI) program as required.
- Follow the agency’s process for managing and addressing customer complaints.
- Uphold patient rights according to service agreements and admission policies.
- Follow agency policies regarding appropriate patient discharge and reasons for discharge.
Requirements
What you’ll need- Must have a CNA or HHA license.
- Must have completed a minimum of 75 hours of training, including an introduction to personal care services.
- Must be free from health problems that may be injurious to the patient, self, and coworkers and present appropriate evidence to substantiate this.
- Ability to comprehend the basics of personal care, housekeeping, and meal preparation.
- Must understand and respect patient needs, including ethics and confidentiality of care.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Ability to walk, stand, stoop, and occasionally lift or turn patients.
