About the role
Key responsibilities & impact- Assess patients’ functional status, including muscle function, endurance, visual coordination, communication skills, self-care, and work capacity
- Evaluate home environments for hazards or barriers to independent living and identify necessary equipment
- Collaborate in developing comprehensive care plans
- Design treatment programs with functional improvement goals and communicate plans to physicians and team members
- Teach or retrain patients in daily activities lost due to illness or injury
- Fabricate splints and instruct patients in adaptive equipment use
- Train patients in the use of prosthetic and orthotic devices
- Maintain accurate clinical records and report patient-condition changes to physicians
- Participate in in-services, case conferences, and team meetings
- Provide health education to patients and families/caregivers with physicians and discharge planners
- Identify needs for additional services and make appropriate referrals
- Confirm daily visit schedules for coordination with other staff
- Supervise Certified Occupational Therapy Assistants according to agency policies and state regulations
Requirements
What you’ll need- Must have a current OT license (state-specific)
- Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association
- Registered with the National Registration Examination of the American Occupational Therapy Association
- Valid BLS certification
- Minimum two years of OT experience; home health experience preferred
- Excellent communication, organizational, and time management skills
- Must have a valid driver’s license and access to a reliable and insured vehicle
- Ability to visit homes that may not be wheelchair accessible
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
- Ability to engage in walking, standing, stooping, and occasional lifting or turning of patients
