About the role
Key responsibilities & impact- Assist in evaluating and developing patient care plans
- Conduct periodic patient reassessments
- Document clinical and daily progress notes for PT review and submission
- Perform therapeutic exercises
- Assist patients with self-help devices
- Instruct patients and families on therapy exercises alongside the PT
- Monitor and report adverse drug reactions or ineffective therapies to the PT
- Observe, document, and report clinical findings to the PT and physician as needed
- Coordinate with PT and nursing staff on patient discharge plans and summaries
- Educate health team members and families to support patient care
- Maintain accurate documentation and comply with agency policies
- Deliver individualized home-based care to patients across Florida
Requirements
What you’ll need- Graduate of an accredited Physical Therapy Assistant program
- Licensed PTA in Florida
- Current BLS certification
- Reliable transportation
- Valid driver’s license and insurance
- One year of home healthcare experience preferred; new graduates welcome to apply
- Experience with electronic medical records is a plus
- Strong physical stamina
- Excellent communication skills
