About the role

Key responsibilities & impact
  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation
  • Develop and implement tailored treatment plans in consultation with physicians and clinicians
  • Adjust treatment plans as needed
  • Collaborate with the interdisciplinary care team to optimize patient outcomes
  • Provide education and support to patients and families/caregivers on therapy programs
  • Maintain accurate clinical records and report changes in patient condition to physicians
  • Supervise Physical Therapy Assistants according to agency policies and state regulations
  • Participate in in-services, case conferences, and meetings
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals
  • Confirm daily visit schedules
  • Communicate patient progress and changes with physicians and care teams
  • Document all visits and progress notes on the same day of service

Requirements

What you’ll need
  • Must have a current PT license (state-specific)
  • Graduate of an accredited physical therapy program
  • Valid BLS certification
  • Minimum one year of PT experience; home health experience preferred
  • Excellent communication, organizational, and time management skills
  • Must have a valid driver’s license and access to a reliable and insured vehicle
  • Ability to visit homes that may not be wheelchair accessible
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
  • Ability to walk, stand, stoop, and occasionally lift or turn patients
  • Proficiency with electronic medical records is a plus
