About the role
Key responsibilities & impact- Perform initial and ongoing patient assessments, including OASIS evaluation and documentation
- Develop and implement individualized treatment plans in consultation with physicians and clinicians
- Adjust treatment plans as needed
- Collaborate with interdisciplinary care teams to optimize patient outcomes
- Educate and support patients, families, and caregivers on therapy programs
- Maintain accurate clinical records and report patient condition changes to physicians
- Supervise Physical Therapy Assistants according to agency policies and state regulations
- Participate in in-services, case conferences, and team meetings
- Identify needs for additional services and make appropriate referrals
- Confirm daily visit schedules
- Communicate patient progress and changes with physicians and care teams
- Document all visits and progress notes on the same day of service
Requirements
What you’ll need- Current state-specific PT license
- Graduate of an accredited physical therapy program
- Valid BLS certification
- Minimum one year of PT experience; home health experience preferred
- Excellent communication, organizational, and time management skills
- Valid driver’s license and access to a reliable and insured vehicle
- Ability to visit homes that may not be wheelchair accessible
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
- Ability to walk, stand, stoop, and occasionally lift or turn patients
- Proficiency with electronic medical records is a plus
