About the role
Key responsibilities & impact- Perform initial and ongoing patient assessments, including OASIS evaluation and documentation
- Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed
- Collaborate with the interdisciplinary care team to optimize patient outcomes
- Provide education and support to patients and families/caregivers on therapy programs
- Maintain accurate clinical records and report changes in patients’ condition to physicians
- Supervise Physical Therapy Assistants in accordance with agency policies and state regulations
- Participate in in-services, case conferences, and meetings to promote coordinated care
- Identify patient and family/caregiver needs for additional services and make appropriate referrals
- Confirm daily visit schedules to coordinate with other staff
- Communicate patient progress and changes with physicians and care teams
- Document all visits and progress notes on the same day of service
Requirements
What you’ll need- Must have a current PT license (state-specific)
- Graduate of an accredited physical therapy program
- Valid BLS certification
- Minimum one year of PT experience (home health experience preferred)
- Excellent communication, organizational, and time management skills
- Must have a valid driver’s license and access to a reliable and insured vehicle
- Ability to visit homes that may not be wheelchair accessible
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
- Ability to engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients
- Proficiency with electronic medical records is a plus
