About the role

Key responsibilities & impact
  • Design and implement patient-specific care plans focused on optimal recovery and overall health
  • Provide nursing care aligned with care plans, including health and psychosocial assessments
  • Monitor patient conditions and coordinate health promotion, prevention, and direct care
  • Maintain accurate clinical records, including required OASIS documentation
  • Communicate with clinical managers and colleagues to ensure high-quality care
  • Train and support patients and their families or caregivers
  • Connect patients with community health resources
  • Collaborate with physicians and discharge planners to provide health education
  • Confirm daily visit schedules and coordinate with staff
  • Participate in the agency’s Quality Assurance and Performance Improvement (QAPI) program
  • Follow the agency’s customer complaint management process
  • Uphold patient rights according to service agreements and admission policies
  • Follow policies regarding patient discharge and reasons for discharge

Requirements

What you’ll need
  • Current state-specific RN license
  • Valid BLS certification
  • Minimum one year of experience in home health or a healthcare facility
  • Familiarity with Medicare and Medicaid guidelines
  • Solid understanding of professional nursing principles and home health documentation
  • Excellent communication, organizational, and time management skills
  • Valid driver’s license and access to a reliable and insured vehicle
  • Ability to visit homes that may not be wheelchair accessible
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
  • Ability to walk, stand, stoop, and occasionally lift or turn patients
