About the role

Key responsibilities & impact
  • Provide support to the Care Management team coordinating care for identified members/patients throughout the healthcare continuum
  • Work closely as a healthcare team member
  • Perform clerical tasks related to overall team functions and activities identified during initial and ongoing assessment and management of members/patients
  • Support patient-focused, whole-person care under the direction of the Supervisor or Manager

Requirements

What you’ll need
  • Medical Terminology coursework or working knowledge of Medical Terminology
  • 1 year of healthcare experience
  • Associate's Degree in a related field (preferred)
  • Medical Assistant Certification (CMA) upon hire (preferred)
  • Proficiency in Microsoft Office applications, including Word and Excel (preferred)
  • Bilingual English/Spanish communication skills (preferred)
  • Experience in managed care or case management (preferred)
  • Experience with McG Care Guidelines, Touchwork, IDX, or Meditech (preferred)
