About the role
Key responsibilities & impact- Provide care transition services for Providence hospitalized patients
- Work with the Providence physician team and hospital discharge planning staff
- Ensure patients are successfully discharged from the hospital without disruption in services
- Perform nursing assessments
- Serve as a liaison among patients, families, physicians, and care teams
- Provide communication, education, and team/family support
Requirements
What you’ll need- Graduate from an accredited school of Nursing
- Washington Registered Nurse License upon hire
- Bachelor's Degree in Nursing (preferred)
- 1 year clinical experience (preferred)
- Experience providing patient and staff education (preferred)
- Experience in conflict resolution (preferred)
