About the role
Key responsibilities & impact- Provide leadership and supervision of the day-to-day clinical and administrative operations of the care management team
- Supervise Care Management staff, including nursing, social work, patient care coordinators, and other non-clinical staff
- Support the organization in achieving its goals
- Collaborate with inpatient and ambulatory clinicians and care teams
- Collaborate with PacMed and PCN leaders
- Support cost-effective, high-quality medical services
Requirements
What you’ll need- Coursework/Training in Nursing; must be an RN
- Upon hire: Washington Registered Nurse License
- 5 years of clinical nursing experience, including acute care hospital and/or ambulatory clinic experience
- Bachelor's Degree in Nursing preferred
- 3 years of case management experience, including care management, care coordination, discharge planning, or population health preferred
- 2 years of progressive supervisory experience preferred
