About the role
Key responsibilities & impact- Conduct on-site and offsite complaint, incident, death, and questionable operations investigations at assigned adult care facilities
- Work as part of a team with an RN or LPN
- Follow NYSDOH-established investigation protocols
- Interview residents, staff, and witnesses
- Conduct records reviews and site inspections
- Complete the investigation closure process
- Document investigation activities in appropriate data systems within required timelines
- Participate in closures and emergency events as assigned
- Attend and complete required training, both in person and remotely
- Professionally represent PCG and the NYSDOH
- Travel regionally within New York State, with occasional statewide travel for facility investigations, training, team meetings, or client engagements
Requirements
What you’ll need- Experienced professional able to work with a Registered Professional Nurse (RN) or Licensed Practical Nurse (LPN)
- Social Worker with a master’s degree in social work or related field and one (1) year social work experience; or a Bachelor’s Degree in social work plus thirty (30) graduate credits; or a bachelor’s degree in a field related to social work with two (2) years of survey or investigation experience
- Pharmacist currently licensed and registered in New York State with one year survey or investigation experience
- Dietitian-Nutritionist currently registered in New York State with one (1) year survey or investigation experience
- Bachelor’s degree in dietetics with satisfactory completion of the requisite dietetic internship in a healthcare facility
- New York State licensed private investigator, currently licensed and registered
- New York State police officer with two (2) years of investigation experience
- Surveyor Minimum Qualifications Test Certification preferred
- Ability to work independently and as part of a team
- Critical thinking, active listening, interviewing, analysis, problem-solving, organizational leadership, and collaboration skills
- Strong verbal and written communication skills
- Proficiency with MS Office: Word, Excel, Outlook, and PowerPoint
- Reliable internet connection
- Valid driver’s license and reliable transportation for travel to on-site facilities and regional offices
- Must obtain Surveyor Minimum Qualifications Test Certification within one year of employment
- PCG does not sponsor newly hired foreign national workers for work authorization, including H-1B sponsorship
