About the role

Key responsibilities & impact
  • Provide nursing oversight, coordination, and hands-on support for individuals receiving residential and mobile crisis services
  • Complete nursing assessments within required timeframes following admission
  • Assess participants’ physical and emotional health needs
  • Provide nursing interventions based on identified health concerns
  • Monitor participant health and recognize changes requiring additional attention
  • Provide face-to-face and telephone support
  • Communicate significant health concerns to the Nurse Manager, Program Director, and appropriate team members
  • Maintain accurate nursing documentation and medication records
  • Assist individuals in learning about medications and treatment
  • Ensure medications are taken as ordered by the physician
  • Maintain medication administration documentation in the electronic MAR
  • Refill non-controlled medications in consultation with the psychiatrist
  • Monitor medication-related concerns and communicate with clinical providers
  • Provide health and medication education supporting recovery and wellness
  • Facilitate residential telepsychiatry appointments using virtual systems
  • Coordinate care with psychiatric providers and behavioral health professionals
  • Participate in medical mobile and mobile crisis interventions
  • Provide nursing support during community-based crisis responses
  • Support stabilization and determine appropriate next steps with crisis professionals
  • Maintain accurate and complete electronic health records
  • Document health concerns, symptoms, interventions, medication information, and changes in participant condition
  • Maintain participant confidentiality
  • Participate in required meetings, trainings, and program activities
  • Complete additional duties as assigned

Requirements

What you’ll need
  • Current Pennsylvania RN or LPN license
  • Current CPR/First Aid Certification
  • Valid Driver’s License
  • Act 33/34 Clearances
  • FBI Clearance as needed
  • Completion of all training required by licensure
  • Required physical examination
  • TB test
  • Hepatitis B titer
  • Ability to work in both residential and community settings
  • Skilled in medication administration and documentation
  • Ability to assess physical and emotional health needs
  • Calm and professional during crisis situations
  • Comfortable collaborating with psychiatric and behavioral health professionals
  • Ability to stand, sit, and walk throughout the workday
  • Ability to climb stairs, reach, bend, kneel, stoop, crouch, push, and pull as required
  • Ability to occasionally exert up to 50 pounds of force, frequently exert up to 20 pounds, and regularly exert up to 10 pounds
  • Ability to perform essential job functions with or without reasonable accommodation
