About the role

Key responsibilities & impact
  • Provides proactive case coordination and patient navigation, including triage, utilization management, algorithm management, and resource management
  • Organizes services across the continuum from pre-registration through discharge to support optimal outcomes, continuity, quality of care, cost reduction, and patient satisfaction
  • Reviews medical records with referring providers and applies utilization review criteria to assess admission, continued stay, and level of care
  • Interviews patients and families regarding social, emotional, and financial factors affecting health status
  • Responds to referrals and collaborates with patients, families, physicians, hospital staff, and community agencies
  • Develops post-hospital plans, initiates referrals, and coordinates continuing care and resource support
  • Identifies utilization problems, monitors clinical pathways and variances, and communicates with physicians and Physician Advisors
  • Maintains current information on community resources, third-party payors, managed care rules, regulations, and procedures
  • Maintains timely medical records, logs, computer entries, statistics, worksheets, and patient documentation
  • Communicates with physicians and coders and identifies case mix issues
  • Provides consultation and education to medical and hospital staff regarding case management, resource management, discharge planning, and complex medical/legal issues
  • Assists in developing new services and policies aligned with patient needs and the strategic plan
  • Refers potential quality problems to appropriate administrative, medical, quality, risk management, infection control, and hospital departments
  • Consults with clinical social workers regarding complex psychosocial and financial needs
  • Collects quality indicators and clinical pathway variance data and participates in improvement teams
  • Develops and maintains relationships with physicians, center leadership, hospital staff, coworkers, and internal stakeholders
  • Provides culturally sensitive services responsive to patient and family beliefs, attitudes, language, and behaviors
  • Ensures adherence to Unity processes, procedures, and standards

Requirements

What you’ll need
  • Graduate of an approved school of nursing
  • Minimum of five years of experience in a health care setting
  • Minimum of two years of experience in an acute care setting
  • Current licensure as an RN in the state of New York
  • Associate of Science in Nursing required
  • BLS - Basic Life Support - American Heart Association (AHA)
  • Prior experience in palliative care strongly preferred
  • Ability to provide care in a culturally sensitive manner
  • Ability to maintain strict confidentiality
  • Ability to comply with Unity processes, procedures and standards
  • Ability to work Monday-Friday days
  • Ability to perform light clinical duties, including frequent standing and walking and occasional lifting of objects up to 20 lbs.
