About the role
Key responsibilities & impact- Conduct initial patient assessments
- Establish plans of care and provide prescribed treatments and demonstrated nursing abilities
- Coordinate the delivery of 24-hour care
- Counsel and guide individuals and families toward self-help in recognizing and solving physical, emotional, and environmental problems
- Conduct regular patient reassessments
- Maintain complete and accurate clinical documentation
- Supervise and serve as a resource to all home care staff
- Deliver coordinated, compassionate care to ElderONE PACE patients in their homes
- Work collaboratively within a mission-driven team
Requirements
What you’ll need- Nursing Diploma required
- Associate of Science in Nursing (Required)
- License and current registration to practice as Registered Professional Nurse in New York State
- Current NYS Driver’s license
- Availability of motor vehicle
- BLS - Basic Life Support - American Heart Association (AHA)
- Excellent ability to follow direction and be flexible to change
- Exceptional communication and interpersonal skills
- Compassionate, warm and patient focused
- Bachelor's degree of Science in Nursing preferred
- 1 year of experience working with a frail or elderly population preferred
- 6 months-1 year experience conducting UAS-NY Community Assessment preferred
- Ability to frequently drive and provide mobile clinical care across settings
- Ability to repeatedly enter and exit vehicles and occasionally lift and carry equipment weighing up to 100 lbs. or more
- Physical readiness and flexibility for extended or on-call schedules
