About the role

Key responsibilities & impact
  • Assist the case management team with delivery of clinical case management services to assigned patient populations
  • Provide supplemental program coordination and operational direction for the AccessHealth Tri-County Network program under the Program Director’s supervision
  • Conduct outreach, monitoring, service planning, financial resource planning, case coordination, advocacy, education, and patient follow-up
  • Serve as the point of contact for patients’ care coordination across hospital departments and community partners
  • Participate in ongoing departmental quality improvement as a patient safety advocate
  • Meet with the Community Care Navigation Team to discuss patient status and conditions, provide guidance, and evaluate effectiveness
  • Provide teammate guidance with the Program Manager when the Program Director is absent
  • Perform daily casework for assigned patients, including completing applications and developing care plans
  • Help develop and maintain a network of human services and community resources
  • Document findings, actions, and plans according to Access Health guidelines and standards
  • Input data into patient charts and assist with data reports
  • Assist with applications for Medicaid, Medicare, AIDS drug assistance, ACA marketplace plans, prescription assistance, charity, and other insurance programs
  • Facilitate collaborative, non-duplicative case coordination and timely communication between agencies
  • Maintain strict confidentiality of client data and comply with information-release policies
  • Perform other required duties and responsibilities

Requirements

What you’ll need
  • Bachelor’s in Social Work, Human Services, Sociology, Psychology, Counseling and Health Education or related field (Required)
  • 5 Years of experience in case management or counseling, preferably in a public health setting or with high-risk populations
  • Experience related to the marginalized population is required
  • Related unpaid and internship experience is accepted
  • Ability to master Access Health South Carolina’s software programs, including CareScope, in a Windows-based environment
  • Ability to master the federal insurance exchange and Benefits Bank
  • Capabilities of analyzing data or information
  • Proficient in case management
  • Knowledge of local healthcare options and resources, including mental health
  • Knowledge of community resources and government assistance programs for the underserved
  • Good organizational and time management skills
  • Ability to prioritize patient needs and services
  • Ability to work collaboratively and develop successful partnerships
  • Ability to coordinate care across multiple inpatient and outpatient hospital and community areas
  • Must be an interdisciplinary team player
  • Must be able to perform shift work
  • Must be able to travel by car to off-site locations
  • Long-distance or air travel as needed, not to exceed 10% travel
