About the role

Key responsibilities & impact
  • Support a caseload of 18–19 households within the Diversion First Permanent Supportive Housing program
  • Deliver person-centered, strengths-based case management grounded in trauma-informed care, Housing First, and harm reduction
  • Conduct initial assessments identifying client strengths, resources, challenges, goals, and referrals
  • Develop individualized housing service plans and review/update them monthly
  • Assess physical health, mental health, social needs, education, life skills, and employment readiness
  • Connect clients with housing, employment, healthcare, childcare, benefits, and other mainstream services
  • Coordinate third-party services including emergency food assistance, furniture resources, employment and training, mental health counseling, and substance use treatment
  • Support clients during crises and provide emergency procedures, coping, and de-escalation skills
  • Facilitate social and community integration
  • Develop financial budgets, provide money management education, and ensure timely rent and utility payments
  • Guide clients toward self-sufficiency and housing stability
  • Maintain accurate client records, case notes, housing stability plans, client files, and HMIS documentation
  • Complete quarterly reviews and annual reassessments
  • Participate in case management meetings, staff meetings, and required trainings
  • Collaborate with Shelter House multidisciplinary teams, the Community Services Board, and partner agencies
  • Share relevant client information to ensure continuity of care
  • Ensure compliance with agency policies and contracts
  • Utilize Microsoft Office Suite for communication and documentation
  • Safely operate Shelter House vehicles to transport clients as needed
  • Demonstrate Shelter House core values and perform other duties as assigned

Requirements

What you’ll need
  • A bachelor's degree in human services/related field or commensurate experience
  • 2+ years of case management/direct services/other related experience
  • Strong written and oral communication skills
  • Ability to prioritize competing priorities and make sound judgements
  • Ability to complete tasks while navigating frequent interruptions
  • Ability to deliver creative, resourceful solutions to unique challenges
  • Strong organizational and time management skills
  • Valid Driver’s License and reliable transportation
  • Ability to drive a 15-passenger van
  • Willingness to work evenings and weekends when needed
  • Annual TB Test is required
  • Ability to sit or stand for long periods
  • Ability to lift items weighing 10–20 pounds
  • Preferred: Foreign Language fluency in a language other than English (e.g. Spanish, Farsi, Arabic or Amharic)
  • Preferred: Experience providing services to clients experiencing homelessness, mental health, or substance abuse issues
  • Preferred: Experience utilizing the Homeless Management Information System (HMIS)
