About the role

Key responsibilities & impact
  • Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards
  • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement and promote seamless medication coordination
  • Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate
  • Support quality-improvement and REMS initiatives
  • Support teammates as needed with complex prior authorizations
  • Improve workflows
  • Educate patients
  • Maintain ongoing clinical development
  • Conduct in-person and virtual patient assessments
  • Build relationships with patients and providers while delivering medication management at an academic medical center

Requirements

What you’ll need
  • Hold a Doctor of Pharmacy (PharmD)
  • Must be a licensed pharmacist in Oregon
  • Completion of a PGY2 Ambulatory Care Residency, or completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA), or at least 3 years of ambulatory practice with disease state management
  • Strong communication, organization, and tech skills
  • Empathy, integrity, and a team-first mindset
  • Solid computer skills required, including proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint)
  • Ability to navigate the internet and web-based platforms
  • Data entry and computer skills will be evaluated through an assessment as part of the pre-employment process
  • Must be able to work in Oregon
