About the role
Key responsibilities & impact- Provide integrated, hands-on support to patients and physicians to improve patient care and medication adherence
- Review clinician notes and assessments for prior authorization approvals and care-plan recommendations
- Develop and refine disease- and drug-specific clinical protocols
- Monitor patient adherence using PDC reports, assessments and mobile-platform data, especially for oncology and specialty disease states
- Evaluate EMR data including drug-disease interactions, medication selection, labs, vitals, allergies, prescriptions, renewals and physician communications
- Recommend alternative therapeutic regimens and disease-specific treatments according to guidelines
- Monitor therapies for side effects and medication-related issues; recommend changes to prescribers
- Perform medication therapy management, document interventions and assess adherence rates
- Conduct pharmacist consultations, motivational interviewing, coaching, medication reconciliation, regimen optimization and adherence interventions
- Coordinate home nurse visits and transitions between liaison/PSC teams
- Monitor productivity and report weekly growth statistics
- Oversee technicians’ daily activities and verify delivery coordination, prior authorizations, logs and care-utilization management
- Identify and resolve insurance issues and financial-assistance opportunities
- Supervise support-personnel communications in compliance with HIPAA rules
- Communicate with health professionals regarding patient care
- Train pharmacy staff, students, interns, externs and healthcare professionals
- Manage and update data using database applications
- Support operational improvements, productivity, quality and customer-service standards
- Assist with research projects and peer-review articles
- Identify trends, resolve problems, recommend improvements and implement change
- Participate in process-improvement initiatives
- Maintain confidentiality of sensitive information
- Perform other duties as assigned
Requirements
What you’ll need- Applicant must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree
- Must be a licensed Pharmacist
- 1+ years' experience is strongly preferred but not required
- Additional required licensure must be obtained within a mutually agreed upon timeframe if needed; continued employment is contingent upon meeting these requirements
- Knowledge of specialty medication pharmacology
- Knowledge of local/state/federal pharmacy regulations and DEA requirements
- Strong technical and clinical skills
- Proficiency with Microsoft Word, Excel, PowerPoint, Outlook, Teams, internet and web-based platforms
- Database and reporting tool knowledge
- EPIC/QS1/WILLOW software knowledge is a plus
- Ability to identify barriers to medication adherence and implement and monitor improvement plans
- Ability to motivate and coach patients to achieve better outcomes and identify therapy goals
- Knowledge of complex/chronic disease states and specialty pharmacy therapies preferred
- Strong verbal and written communication skills
- Ability to manage processes and people
- Computer and data-entry skills are assessed during pre-employment
- Must maintain registration and comply with local, state and federal drug laws and applicable state board of pharmacy and DEA regulations
