About the role

Key responsibilities & impact
  • Develop relationships with external Housing and Health Opportunities (H2O) providers to build and support the H2O network
  • Serve as the primary point of contact and subject matter expert on the H2O program for contracted providers
  • Manage a high volume of provider inquiries requiring research, input, and follow-up
  • Document and maintain records of communications with assigned providers
  • Plan and lead regular on-site and/or virtual meetings and educational seminars with contracted providers
  • Educate H2O providers on program requirements and best practices in homelessness services delivery
  • Collaborate with departments or designees to develop training and education
  • Conduct ongoing review of provider performance and engage in program improvement conversations
  • Assist in care coordination for complex cases
  • Plan, organize, prioritize, and work independently with limited supervision
  • Work in the field with community partners independently and autonomously
  • Perform other duties as assigned

Requirements

What you’ll need

  • 3-5 years of experience in Health, Human or Social Services, or related field (Preferred)
  • Bachelor's Degree in business, social work, or related field (Preferred)
  • Standard Business Hours, Monday-Friday 8 a.m. to 5 p.m.
  • Ability to maintain a stable and secure private home internet with minimum 50Mbps download, 5Mbps upload, and average jitter less than 20% of latency
