About the role
Key responsibilities & impact- Review the daily caseload and prepare technology and tools for patient care
- Navigate the daily schedule using modern technology

- Build welcoming connections with diverse patients

- Provide patients with educational information about health and well-being
- Evaluate vital signs with Sprinters
- Arrange blood draws and address patient-specific needs
- Provide care coordination and management
- Deliver preventive care interventions and medication management
- Arrange specialist referrals and connect patients with community resources
- Document clinical findings
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Louisiana
- Prior experience performing wellness visits
- Professionalism, integrity, accountability, confidentiality, care, and compassion
- Willingness to perform last-minute problem solving and out-of-the-box thinking
- Comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
- Must be able to work remotely in the United States
