About the role
Key responsibilities & impact- Conduct virtual adult and/or pediatric wellness visits, health assessments, and related healthcare services
- Review caseload and prepare technology for patient service
- Navigate the daily schedule using technology
- Partner with clinical in-home team members visiting patients’ homes
- Build welcoming connections with diverse patients
- Collect relevant patient information and perform services
- Conduct health risk assessments, medication reviews, and cognitive screenings
- Provide patients with educational information about their health and well-being
- Collaborate with Sprinters to evaluate vital signs and arrange blood draws
- Carry out tasks addressing patients’ specific needs
- Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation of findings
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Louisiana
- Prior experience performing wellness visits
- Professionalism, integrity, accountability, confidentiality, care, and compassion
- Willingness to perform last-minute problem solving and out-of-the-box thinking
- Technologically savvy and comfortable using laptops or mobile devices for charting
- Comfortable using HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Must pass national background check and valid clinical license search
- Must complete required application questions regarding state licensure, wellness-visit experience, Medicaid enrollment, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification
