About the role
Key responsibilities & impact- Review caseload and prepare technology for patient services
- Navigate the daily schedule using technology
- Partner with clinical in-home team members visiting patients’ homes
- Build welcoming connections with diverse patients
- Conduct virtual adult and/or pediatric wellness visits
- Perform health risk assessments, medication reviews, and cognitive screenings
- Provide patients with educational information about health and well-being
- Evaluate vital signs and arrange blood draws with Sprinters
- Provide care coordination and management
- Deliver preventive care interventions, medication management, specialist referrals, community-resource referrals, and documentation
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Louisiana
- Prior experience performing wellness visits
- Professionalism, integrity, accountability, confidentiality, care, and compassion
- Willingness to perform last-minute problem solving and out-of-the-box thinking
- Technologically savvy and comfortable using laptops, mobile devices, and HIPAA-secure messaging apps
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
