About the role

Key responsibilities & impact

Conduct virtual adult and/or pediatric wellness visits from patients’ homes
Perform health assessments, health risk assessments, medication reviews, cognitive screenings, and wellness services
Provide patients with educational information regarding their health and well-being
Partner with clinical in-home team members (Sprinters) visiting each patient’s home
Evaluate vital signs and arrange blood draws
Coordinate and manage preventive care interventions, medication management, specialist referrals, community resources, and patient care
Document clinical findings
Review caseloads and prepare technology before starting the day
Navigate daily schedules using modern technology
Provide personalized, patient-centric care and maintain high-quality standards

Requirements

What you’ll need

Board Certified as a Family Nurse Practitioner
Active Family Nurse Practitioner license in Louisiana
Prior experience performing wellness visits
High levels of professionalism, integrity, accountability, confidentiality, care, and compassion
Willingness to perform last-minute problem solving and out-of-the-box thinking
Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps
Strong written and verbal communication skills
Ability to work independently or in a team environment
Must pass a national background check and valid clinical license search
Must provide high-quality health services
Required application information includes states licensed as a Nurse Practitioner, Medicare and/or Medicaid Annual Wellness Visit experience, Medicaid enrollment status, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification
