About the role
Key responsibilities & impact- Conduct virtual adult and/or pediatric wellness visits, health assessments, and related services
- Review caseload and prepare technology for patient care
- Navigate the daily schedule and partner with clinical in-home team members visiting patients’ homes
- Build welcoming connections with diverse patients and collect relevant information
- Conduct health risk assessments, medication reviews, cognitive screenings, and patient education
- Work with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs
- Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in Georgia
- Prior experience performing wellness visits
- Professionalism, integrity, accountability, confidentiality, care, and compassion
- Willingness to perform last-minute problem solving and out-of-the-box thinking
- Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
- Virtual experience as a Family Nurse Practitioner is assessed in the application
- Experience with Medicare and/or Medicaid Annual Wellness Visits is assessed in the application
- Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner is assessed in the application
