About the role
Key responsibilities & impact- Review the daily caseload and prepare technology for patient service
- Navigate the daily schedule using Sprinter Health technology
- Partner with clinical in-home team members visiting patients' homes
- Connect with diverse patients and collect relevant information
- Conduct virtual adult and/or pediatric wellness visits and health assessments
- Perform health risk assessments, medication reviews, and cognitive screenings
- Provide patients with educational information about health and well-being
- Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs
- Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Louisiana
- Prior experience performing wellness visits
- High levels of professionalism, integrity, accountability, confidentiality, care, and compassion
- Willingness to perform last-minute problem solving and out-of-the-box thinking
- Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Must pass a national background check and valid clinical license search
- Must be able to provide services in the listed Kentucky locations
