About the role

Key responsibilities & impact
  • Administer skilled nursing care to patients intermittently in their residences according to physician orders and plans of care
  • Provide and oversee care for assigned patients within the scope of RN licensure
  • Review service requests and determine efficient routes, timing, and delivery requirements
  • Ensure supplies are available for off-site visits
  • Drive safely to patient homes, assisted living and long-term care residences, jails, and other designated locations
  • Participate in interdisciplinary case management to optimize clinical outcomes
  • Meet Home Health productivity standards
  • Identify patients appropriate for Lifeline
  • Respond to after-hours calls, assess medical needs, conduct visits, or direct patients to appropriate care
  • Assess patient conditions and needs, establish outcomes, implement nursing actions, collaborate with other disciplines, and evaluate progress
  • Calculate dosages and administer medications accurately
  • Document patient care in compliance with hospital policies and regulatory agencies
  • Participate in quality improvement, departmental activities, and professional development
  • Communicate effectively with caregivers, medical staff, patients, and families
  • Manage the patient experience by coordinating interdisciplinary team roles
  • Support organizational vision, mission, values, and Value Improvement Practice principles
  • Maintain safe, professional, confidential, and ethical care environments
  • Deliver timely, efficient, accurate customer service and patient care
  • Create healing environments and therapeutic relationships with patients and caregivers

Requirements

What you’ll need
  • Education as required for licensure/certification
  • Current valid Oregon RN license
  • AHA Basic Life Support for Healthcare Provider certification
  • Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements
  • Ability to travel to patient residences with own vehicle
  • 1 year of relevant nursing experience using assessment skills and critical thinking regularly
  • Ability to wear required Personal Protective Equipment (PPE)
  • Excellent communication skills and ability to interact with a diverse population
  • Excellent organizational and multitasking skills and ability to work independently
  • Strong teamwork and collaboration skills
  • Strong analytical, problem-solving, and decision-making skills
  • Basic to intermediate experience with electronic medical records systems and MS Office
  • Ability to meet physical requirements including standing, walking, keyboard operation, speaking, hearing, and lifting/carrying/pushing/pulling
  • Ability to work scheduled days, which may include weekends and holidays
