About the role
Key responsibilities & impact- Complete assigned virtual nursing activities, including patient education, documentation, and care coordination
- Assist with care progression, discharge readiness, and escalation of patient care concerns
- Support evidence-based care delivery and continuity of care
- Conduct virtual patient interactions and promote patient and family engagement
- Reinforce plans of care, discharge instructions, and medication education
- Support patient safety initiatives and escalate concerns to appropriate care team members
- Partner with bedside nurses, providers, care management, and interdisciplinary team members
- Maintain timely communication to support high-quality patient care
- Serve as a resource for virtual nursing workflows and patient education
- Utilize virtual care platforms, Epic, and communication technologies to support clinical workflows
- Maintain confidentiality and comply with Network and departmental policies
- Demonstrate service excellence, performance improvement, and financial responsibility
- Demonstrate competency in assessment, treatment, growth and development, and age-appropriate communication
Requirements
What you’ll need- Graduate of an accredited School of Nursing
- Current Registered Nurse license in PA and NJ and/or Multistate Nursing license
- BLS certification required
- Clinical nursing experience preferred
- Experience with Epic and virtual care technologies preferred
- Ability to sit for extended periods while utilizing multiple computer monitors and communication platforms
- Visual and auditory ability sufficient to monitor patient interactions and clinical information
- Ability to communicate effectively through video, telephone, and electronic communication systems
- Must submit full legal name and current home address
- Include employment history for the past seven years, including present employer
