About the role
Key responsibilities & impact- Provide whole-person 1:1 coaching to drive behavior changes that improve clinical outcomes and overall health status
- Foster positive relationships with members and provide an excellent member experience
- Empower members to self-manage chronic conditions
- Assess individual member needs and deliver appropriate education and coaching
- Use Motivational Interviewing for a member-centered approach
- Attend mandated meetings and complete required online and/or in-person training
- Follow scheduling best practices to meet coaching utilization and member needs
- Participate in process improvement
- Mentor less-experienced coaches through formal or informal support, feedback, and answers to questions
- Support development of education handouts, videos, podcasts, and client-facing presentations
- Stay current on clinical best practices and share clinical expertise across the team and company
- For Diabetes Management coaching, provide 1:1 diabetes education and coaching based on ADCES 7 Self-Care Behaviors and evidence-based recommendations
- Facilitate live diabetes-focused group Learning Events as needed
- Allocate up to 30% of time to liaison, project, or cross-functional responsibilities as approved by management
Requirements
What you’ll need- Minimum of a bachelor’s degree
- Must possess and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES)
- Maintain the required 75 CEUs in 5 years to maintain CDCES certification
- Apply evidence-based principles to enhance coaching practice
- Review research findings and apply them to practice
- Interpret and incorporate the ADCES 7 DSME/T education model into practice
- Strong background knowledge of diabetes science and latest treatment options
- At least 3 years of professional experience in the healthcare field
- Provide quality member education and coaching
- Assess individual member needs, including skills, knowledge, barriers, and motivation
- Use a coaching approach congruent with Motivational Interviewing
- Advanced degree in a health-related field, bilingual Spanish/English, and NBC-HWC certification are desirable
- Must be currently authorized to work in the United States without visa sponsorship; Teladoc Health will not sponsor or transfer employment work visas
