About the role
Key responsibilities & impact- Manage community engagement initiatives supporting successful transitions to community living
- Coordinate resource networks, gather stakeholder input, identify access barriers and service gaps, and improve outreach, referrals, member experience, and community stability
- Participate in community, client, and state forums; coordinate engagement plans, issue resolution, performance monitoring, and cross-functional activities
- Build and maintain relationships with community organizations, providers, agencies, advocates, members, families, caregivers, and other stakeholders
- Coordinate culturally responsive outreach, listening sessions, focus groups, advisory activities, and community forums
- Monitor participation, referral outcomes, partner responsiveness, resource capacity, access barriers, and expenditures
- Ensure activities meet contractual and reporting requirements and communicate results, emerging needs, and recommended actions
- Coordinate with interdisciplinary teams supporting outreach, member engagement, resource navigation, and transition services
- Promote member-centered, culturally responsive, and community-informed practices
- Perform other duties and special projects as assigned
- Support the Illinois Tailored Care Management Program, helping people with complex health and social needs transition from nursing facilities and SMHRFs to community-based living
Requirements
What you’ll need- Bachelor’s degree in healthcare, human services, community development, or a related field, or equivalent training and experience
- Five years of relevant experience
- Experience coordinating community engagement, building stakeholder relationships, and managing programs or projects
- Strong written, verbal, and facilitation skills
- Ability to travel locally and overnight up to 50%
- Master’s degree preferred
- Experience with Medicaid or other government-sponsored healthcare programs, community-based services and supports, behavioral health or social service systems, community transitions, and addressing health-related social needs preferred
- Bilingual English/Spanish skills preferred
- Relevant community engagement or program management certification preferred
- Must not require sponsorship for work authorization now or in the future
