About the role
Key responsibilities & impact- Connect individuals with community resources and programs and provide non-clinical support to care managers
- Develop and maintain relationships with community resources, facilities, state agencies, and community entities
- Make referrals for transition services based on members’ person-centered service plans
- Follow up and monitor resource referrals and coordinate activities within established timelines
- Guide members to appropriate benefit information
- Maintain an accurate, comprehensive resource library for members in Cook County and across Illinois as needed
- Collaborate with care managers, healthcare professionals, and social service agencies to coordinate healthcare and transition services
- Escalate issues through appropriate channels and established hierarchical protocols
- Collaborate across disciplines to promote enrollee compliance and appropriate use of community resources
- Assist members, families, and community supports with personal and environmental resources
- Perform miscellaneous duties as assigned
Requirements
What you’ll need- High School diploma or equivalent with 3 years of experience in community-based services, or Bachelor’s degree in social work or related field
- Certified care and/or case manager, or Licensed Practical Nurse (LPN), or health coach with at least one year of experience, or Community Health Worker certification through a college-level program or ability to obtain certification
- In-depth knowledge of social determinants of health and their impact on healthcare
- Valid Illinois driver’s license
- Reliable transportation
- Bilingual in English and Spanish preferred
- Behavioral Health knowledge preferred
- Knowledge of confidentiality practices and HIPAA and PHI requirements
- Ability to learn software applications including database management and report development
