About the role

Key responsibilities & impact
  • Provide face-to-face and/or virtual Specialized Transition Services to members seeking services within community settings
  • Collaborate with Care Managers, health care providers, and ancillary service providers to implement appropriate care plan support
  • Facilitate member transition and community living, housing navigation, resources, and funding options
  • Review person-centered care plans regarding services needed based on set guidelines
  • Document transition support activity results and update care plans as needed
  • Provide ongoing education
  • Communicate with health care providers to ensure appropriateness of care
  • Perform miscellaneous duties as assigned

Requirements

What you’ll need
  • Four-year degree in a health care or related field and/or relevant experience
  • Valid driver’s license
  • Up to 95% local travel
  • Case Management experience preferred
  • Bilingual in English and Spanish a plus
  • Telligen will not provide sponsorship for work authorization now or in the future
