About the role
Key responsibilities & impact- Deliver comprehensive services to adults with serious mental illness, co-occurring substance use disorders, and criminal legal system involvement through Forensic Assertive Community Treatment (FACT)
- Provide at least 80% of medication management and health services in the community
- Administer oral and intramuscular medications
- Monitor medication side effects and adherence
- Perform phlebotomy and health assessments
- Coordinate consumers’ health care services
- Provide health and medication psychoeducation
- Assess consumers’ mental statuses
- Rotate on the team’s after-hours emergency line
- Complete progress notes, nursing assessments, incident reports, and other documentation according to OMH, DOHMH, and Medicaid requirements
- Collaborate with the FACT prescriber and LPN on medication management and health care services
- Direct the LPN regarding medication management, health education, counseling, and psychoeducation
- Coordinate care with families and community service providers, including hospitals, housing providers, shelters, criminal justice personnel, AOT, and primary care providers
- Participate in daily team meetings, clinical meetings, and monthly nurses meetings
- Use motivational interviewing and culturally appropriate, recovery-oriented, and trauma-informed approaches
- Perform other duties as required by the Team Leader and senior clinical leadership
Requirements
What you’ll need- Registered Nurse (RN) with current New York State license
- At least three years of experience working with adults (ages 18 and up) in psychiatric nursing, substance use treatment, social service settings caring for individuals experiencing homelessness, justice involvement, or limited social support
- Extensive knowledge of community treatment, support services, and resources
- Highly organized and self-motivated
- Proficient in computers
- Fluency in Spanish preferred
