About the role

Key responsibilities & impact
  • Establish relationships with individuals accessing the service that encourage self-determination and participation.
  • Provide case management support in accordance with TSA case management models and standards of practice.
  • Complete thorough assessments inclusive of needs, risks and vulnerabilities.
  • Provide ongoing assessment from initial intake to exit.
  • Manage individual case planning collaboratively, including interventions addressing individual needs, goals and barriers.
  • Support ADF veterans and veteran families to secure housing, improve social inclusion and achieve housing stability across Queensland.
  • Work effectively with team members to provide a comprehensive service aligned with The Salvation Army’s vision, mission and values.

Requirements

What you’ll need
  • Tertiary qualification in Community Services, Social Work or related field.
  • Relevant experience in a social service environment including in client assessment and case management.
  • QLD Working with Children Check.
  • Driver’s Licence.
  • First Aid Certificate.
  • Applicants for all roles require a mandatory Nationally Coordinated Criminal History Check.
  • Ability to provide person-led case management incorporating resilience, trauma informed and strengths-based practices.
  • Ability to support ADF veterans and veteran families across Queensland.
