About the role

Key responsibilities & impact
  • Work with assigned families and individuals in a housing program toward a housing plan
  • Maximize income, file taxes, obtain identification, and connect clients with community supports
  • Respond to calls and referrals from community agencies according to caseload
  • Place individuals and families into permanent leased units based on individual needs
  • Assess and screen participant and family needs using an acuity tool and other resources
  • Complete Social & Community Housing applications (HATCH) and submit required documents, or keep existing applications active
  • Assess clients for domestic violence experiences and refer them to appropriate programs
  • Assist participants with moves into independent accommodations and provide neighborhood orientation
  • Communicate with social housing providers, administrators, community partners, and private landlords
  • Report suspected or witnessed child abuse or neglect to the appropriate child welfare agency (CAS)
  • Conduct home or shelter visits to provide life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration, grocery shopping, cleaning, and related supports
  • Maintain accurate records according to Wesley expectations
  • Support approximately one 24-hour intake shift per month
  • Perform other duties as assigned

Requirements

What you’ll need
  • Post-secondary degree/diploma in Human Services or related field
  • Must qualify to register with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Experience working with adults and families who may be experiencing homelessness, mental health and substance use issues
  • Ability to provide non-judgmental supports that encourage reducing barriers and empowerment
  • Ability to work flexible shifts including day, evening and weekend hours
  • Ability to fulfill 24-hour intake responsibilities
  • Ability to meet the travel expectations of the position
  • Valid driver’s license and reliable access to a vehicle
