About the role

Key responsibilities & impact
  • Conduct monthly CCM phone calls with the assigned practice
  • Make 30–50 outbound calls per day
  • Complete required patient interactions to meet CCM service goals
  • Educate patients on chronic conditions and care plans
  • Coordinate care with provider offices, pharmacies, and community resources
  • Identify gaps in care and social determinants of health
  • Create and update personalized care plans in the EMR
  • Accurately document all patient interactions
  • Maintain HIPAA compliance and patient confidentiality
  • Spend 7–8 hours per day on the phone
  • Work a fixed full-time schedule of 40 hours per week, Monday–Saturday, 9:30 AM–6:00 PM in the supported time zone
  • Meet monitored productivity and quality metrics

Requirements

What you’ll need
  • Minimum 1–3 years of experience as a CMA/RMA or LPN
  • Active certification or multistate license for LPNs
  • Strong verbal and written communication skills in English
  • Comfortable being on the phone most of the day
  • Experience with EMR/EHR systems
  • Organized, reliable, and able to work independently in a remote setting
  • Preferred: geriatric patient experience
  • Preferred: care coordination or case management experience
  • Preferred: strong verbal and written communication skills in Spanish
  • Private, quiet home office with no interruptions
  • Reliable high-speed internet (100 Mbps or higher)
  • Second monitor required
