About the role
Key responsibilities & impact- Provide professional nursing care to hospice clients as prescribed by the physician and/or requested by the client/family
- Conduct client assessments, care planning, therapeutic interventions, and supervise client care and outcomes as hospice case manager
- Collaborate with physicians and Hospice team members to anticipate and manage patient symptoms
- Assist with identifying and addressing patient, family, and caregiver grief/loss issues
- Provide functional support to Hospice Aides and LPN/LVNs
- Inform clients/caregivers about the agency, available care/services, payment obligations, responsibilities, and rights
- Develop and implement individualized, outcome-focused Plans of Care
- Refer clients to other disciplines and services and coordinate care
- Assess clients using interviews, observation, and evaluation; formulate nursing interventions and recommendations
- Report changes in client condition and obtain orders for treatment-plan changes
- Re-evaluate and update Plans of Care based on patient goals and progress
- Assess patient and family learning needs and provide education on disease process, self-care, end-of-life care, ethical concerns, and patient goals
- Respond to changes in patients’ physical, psychological, or spiritual conditions
- Maintain knowledge of pain and symptom management using evidence-based protocols and standing guidelines
- Prepare patients and families for the expected disease process and provide anticipatory guidance about death and dying
- Document assessments, visits, medications, treatments, coordination, progress, and physician/provider contacts in the point-of-care information system
- Submit client data timely for inclusion in records
- Document collaborative contacts and ensure documentation and orders are current and timely
Requirements
What you’ll need- Graduate of an approved Nursing education program
- Current Registered Nurse licensure in the state of practice
- Minimum of one (1) year experience as a professional acute care nurse
- Home healthcare/hospice experience preferred
- Strong communication and interpersonal skills, with a special emphasis on grief/loss management
- Current Driver’s license and reliable transportation to and from work site
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program
