About the role
Key responsibilities & impact- Provide primary care services under established medical protocols
- Diagnose and treat patients using evidence-based practices
- Prescribe medications in consultation with the Medical Director as needed
- Collaborate with healthcare professionals to ensure holistic patient care
- Participate in community health fairs and outreach events 3-4 times a year
- Deliver age-appropriate care, primarily for adults (18-65) and geriatric patients (65+), with potential to serve pediatric patients
- Coordinate care with medical and social services
- Support accessible healthcare through community outreach
Requirements
What you’ll need- Graduation from an NLN-approved MSN Nurse Practitioner program or a certified Nurse Practitioner program with three years of NP experience
- Georgia RN license required
- Two years of RN experience preferred
- Experience working with diverse and underserved populations preferred
- Ability to diagnose and treat patients in health and illness using health promotion, disease prevention, and therapeutic interventions
- Knowledge of health problems, issues, and needs of the specific target populations
- Bilingual (English/Spanish) proficiency strongly preferred for the Chamblee Clinic
