About the role
Key responsibilities & impact- Deliver primary and preventive healthcare services in community-based and outdoor settings
- Assess, diagnose, and treat acute and chronic medical conditions among individuals experiencing homelessness
- Perform health screenings, wound care, medication management, and chronic disease monitoring
- Collaborate with social workers, community health workers, behavioral health professionals, and community partners to address social determinants of health
- Connect patients to housing resources, behavioral health services, specialty care, substance use treatment, and other supportive services
- Develop individualized care plans to improve health outcomes and reduce barriers to care
- Document patient encounters and coordinate follow-up care across Mercy Care clinics and community partners
- Participate in outreach activities focused on engaging hard-to-reach populations and establishing continuity of care
- Advocate for patients and serve as a trusted healthcare partner for individuals with limited access to healthcare services
Requirements
What you’ll need- Graduation from an NLN-approved MSN Nurse Practitioner program or a certified Nurse Practitioner program with three years of NP experience
- Georgia RN license required
- Two years of RN experience preferred
- Experience working with diverse and underserved populations preferred
- Ability to diagnose and treat patients in health and illness using health promotion, disease prevention, and therapeutic interventions
- Knowledge of health problems, issues, and needs of the specific target populations
