About the role

Key responsibilities & impact
  • Serve as the first point of contact for incoming transfer, admission, and consultation requests
  • Collect and verify demographic, referral, and operational information from sending facilities, EMS agencies, and providers
  • Manage multi-line phone activity, prioritize calls, and minimize delays
  • Document call details and required information in electronic health records and transfer management systems
  • Route calls and information to Patient Logistics Clinicians, RNs, Charge RNs, or operational team members
  • Support EMTALA-sensitive processes through complete intake documentation and immediate escalation of red flags
  • Coordinate non-clinical transport logistics, including contacting EMS agencies, confirming availability, monitoring ETA updates, and communicating status changes
  • Demonstrate Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions

Requirements

What you’ll need
  • Health care experience required
  • High school diploma or GED required
  • Associate’s Degree or equal experience preferred
  • Comfortable operating in a collaborative, shared leadership environment
  • Personal presence characterized by honesty, integrity, and caring
  • Ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health and MercyOne
