About the role

Key responsibilities & impact
  • Provide professional physical therapy services to home health clients according to physician orders and established plans of care
  • Perform comprehensive assessments
  • Develop and implement individualized treatment plans
  • Deliver preventive, therapeutic, and rehabilitative interventions in the home setting
  • Coordinate care with physicians and the interdisciplinary team

Requirements

What you’ll need
  • Graduate of an APTA-approved Physical Therapy program
  • Licensed Physical Therapist in the state of practice
  • Minimum one (1) year of experience or completion of a 6–8 week preceptorship
  • Home health experience preferred
  • Ability to work independently with minimal supervision
  • Commitment to maintaining clinical competence, organizational policies, and ethical standards
  • Valid driver's license and reliable transportation required
  • Ability to lift up to 35 lbs and assist with patient mobility
  • Frequent standing, walking, bending, and positioning
  • Adaptability to changing schedules and environments
  • Travel to client homes and Trinity Health locations required
