About the role

Key responsibilities & impact

Assess and care for patients using video conferencing technology.
Support seamless admissions, discharges, and patient education within the acute care unit.
Collaborate, communicate, and facilitate optimal provision of care through computerized and/or digital platforms.
Assess, plan, implement, coordinate, monitor, and evaluate patients' plans of care from admission to discharge.
Use clinical knowledge, critical thinking, case management, and adult education principles to coordinate and support care plans.
Ensure effective and efficient transitions of care across the continuum.
Monitor department-specific quality metrics.
Ensure complete and accurate documentation in patient records.
Serve as mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

What you’ll need

Current Iowa RN license
Minimum five (5) years of equivalent experience in health condition management in an adult acute care clinical setting with direct patient care experience within the last 10 years.
BSN preferred
Availability for full-time night shifts from 7 PM - 7 AM
Three 12-hour shifts per week
Ability to deliver patient care through virtual platforms and video conferencing technology
Clinical knowledge, critical thinking skills, and principles of case management and adult education
Ability to collaborate, communicate, and facilitate care through computerized and/or digital platforms
