About the role
Key responsibilities & impact- Assess and care for patients using video conferencing technology.
- Ensure seamless admissions, discharges, and patient education within the acute care unit.
- Collaborate, communicate, and facilitate optimal provision of care through computerized and/or digital platforms.
- Assess, plan, implement, coordinate, monitor, and evaluate patients’ plans of care from admission to discharge.
- Apply clinical knowledge, critical thinking, case management, and adult education principles.
- Coordinate, implement, and support plans of care.
- Ensure effective and efficient transitions of care across the continuum.
- Monitor department-specific quality metrics.
- Ensure complete and accurate patient-record documentation.
- Serve as mentor, clinician, educator, consultant/collaborator, and navigator.
Requirements
What you’ll need- Current Iowa RN license.
- Minimum five (5) years of equivalent experience in health condition management in an adult acute care clinical setting with direct patient care experience within the last 10 years.
- BSN preferred.
