About the role

Key responsibilities & impact
  • Collaborate and facilitate optimal provision of care through virtual daily management using computerized and/or digital platforms
  • Assess, plan, implement, coordinate, monitor, and evaluate patients’ plans of care from admission to discharge
  • Organize care in collaboration with multidisciplinary teams and participate directly or indirectly with patients
  • Use clinical knowledge and critical thinking to coordinate, implement, and support plans of care
  • Ensure seamless, effective, and efficient transitions of care across the continuum
  • Perform chart audits to enhance quality and safety
  • Monitor department-specific quality metrics and ensure complete and accurate patient-record documentation
  • Perform virtual Safety Rounds
  • Serve as clinician, educator, consultant/collaborator, and navigator
  • Support the Holy Cross Health Model of Care, shared governance, nursing quality initiatives, and ANA standards

Requirements

What you’ll need
  • Graduation from an accredited nursing program; basic professional nursing degree required
  • BSN preferred; if BSN or MSN is not present on hire, employee must sign an agreement to obtain one within a specific time frame
  • Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse required
  • Minimum five years of RN experience in an acute care setting, including adult condition management
  • Electronic Medical Record (EMR) experience highly preferred
  • BLS certification
  • Strong interpersonal skills and ability to work collaboratively in a multidisciplinary setting
  • Basic computer proficiency for documentation and communication
  • Ability to lift up to 35 pounds and assist with patient handling and movement
  • Proficiency in English: reading, writing, speaking, and comprehension
