About the role
Key responsibilities & impact- Collaborate and facilitate optimal provision of care through virtual daily management using computerized and/or digital platforms
- Assess, plan, implement, coordinate, monitor, and evaluate patients' plans of care from admission to discharge
- Organize care with the multidisciplinary care team and participate directly or indirectly with patients
- Use clinical knowledge and critical thinking to coordinate, implement, and support plans of care
- Ensure seamless, effective, and efficient transitions of care across the continuum
- Perform chart audits to enhance quality and safety
- Monitor department-specific quality metrics and ensure complete, accurate patient-record documentation
- Perform virtual Safety Rounds
- Serve as clinician, educator, consultant/collaborator, and navigator
- Promote the Holy Cross Health Model of Care and nursing profession
- Facilitate shared governance and comply with ANA standards and Code of Ethics
Requirements
What you’ll need- Graduation from an accredited nursing program; basic professional nursing degree required (BSN preferred)
- If the BSN or MSN degree is not present on hire, the employee must sign an agreement to obtain one within a specific time frame
- Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse required
- Minimum five (5) years of RN experience in an acute care setting, including adult condition management, required
- Electronic Medical Record (EMR) experience highly preferred
- BLS certification
- Strong interpersonal skills and ability to work collaboratively in a multidisciplinary setting
- Basic computer proficiency for documentation and communication
- Ability to lift up to 35 pounds and assist with patient handling and movement
- Proficiency in English: reading, writing, speaking, and comprehension
