About the role

- Manage care and resources to reduce unnecessary utilization.
- Apply clinical expertise and understanding of care management, CMS and NCQA regulatory requirements, and quality measures of care.
- Provide safe and appropriate patient transitions across different levels of care.
- Develop person-centered care goals.
- Identify barriers to health and implement interventions to meet patient goals.
- Develop individualized care plans emphasizing patient self-management and family responsibility.
- Identify and rank the relative importance and urgency of potential problems.
- Plan contingencies and take ownership of problems.
- Engage in individual and group problem-solving.
- Adapt effectively to change and challenging situations.
Requirements
What you’ll need- Current unrestricted California RN license.
- BSN Degree.
- 5 years of Case Management experience.
- Strong problem solving skills; ability to analyze problems and formulate plans, solutions and a course of action given changing circumstances.
- Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously.
- Strong ability to prioritize.
- Proficiency in Windows computer functions, computer navigation, and computerized patient care systems.
- Competence with internet/intranet navigation and Microsoft Explorer, Excel, Word, PowerPoint, MS Teams, Care-Connect/EPIC, Zoom, ICAP, Right-Fax, AIDIN, and JABRA Headset.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to articulate clinical and non-clinical information to individuals of all levels of understanding.
- Ability to work flexibly according to the availability of office personnel, patients, and required resources.
- MSN degree preferred.
- CCM or ACM preferred.
- Final candidate accepting employment must disclose qualifying misconduct-related administrative or judicial decisions or appeals within the last seven years.
