Key responsibilities & impact
  • Function as a clinician, advocate, and educator to achieve optimal clinical and quality outcomes
  • Manage care and resources to reduce unnecessary utilization
  • Apply clinical expertise and knowledge of care management, CMS and NCQA regulatory requirements, and quality measures
  • Provide safe and appropriate patient transitions across different levels of care
  • Develop person-centered care goals
  • Identify barriers to health and implement interventions to meet patient goals
  • Develop individualized care plans emphasizing patient self-management and family responsibility
  • Identify and rank the relative importance and urgency of potential problems
  • Plan contingencies and engage in individual and group problem solving
  • Adapt effectively to change while managing challenging situations

Requirements

What you’ll need
  • Current unrestricted California RN license
  • BSN Degree
  • 5 years of Case Management experience
  • Strong problem solving skills; ability to analyze problems and formulate plans, solutions and a course of action given changing circumstances
  • Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously
  • Strong ability to prioritize
  • Proficiency in Windows computer functions, computer navigation, and computerized patient care systems
  • Competence using internet/intranet, browser software, Microsoft Explorer, Excel, Word, PowerPoint, MS Teams, Care-Connect/EPIC, Zoom, ICAP, Right-Fax, AIDIN, and JABRA Headset
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to articulate clinical and non-clinical information to individuals of all levels of understanding
  • Ability to work flexibly according to office personnel, patient, and resource availability
  • Final candidate must disclose qualifying misconduct-related administrative or judicial decisions or appeals from the last seven years as a condition of employment
