About the role
Key responsibilities & impact- Collaborate with the Program Director to ensure the program meets or exceeds all accreditation requirements
- Conduct outreach to individuals, communities, service providers, and groups
- Identify and recruit individuals, families, and community groups to services and systems
- Educate health and social service systems and providers about community perspectives and cultural norms
- Facilitate cross-cultural communication among individuals, communities, and health/social service system workers
- Translate and culturally adapt resources and educational materials
- Coordinate care and provide case management support
- Assist individuals and communities in navigating health and social service systems
- Participate in community-level assessments, including windshield surveys and community asset mapping
- Present at local agencies and community events to share information and educate individuals and communities about health and social service concerns and resources
Requirements
What you’ll need- High School Diploma/GED plus two (2) years of general experience in community outreach, education, case management, or administration
- Must enroll in a certified CHW Training Program within 45 days of employment
- Must reside in or be willing to commute to Jefferson County
- Reliable transportation, a working cell phone, and internet access are essential
- Strong interpersonal and communication skills
- Experience in community outreach, public health, or related fields
- Ability to mentor and supervise team members (CHW III)
- Proficiency in case management and group education
- Knowledge of health disparities and culturally appropriate outreach strategies
- Organizational and multitasking abilities
- Proof of legal authority to work in the United States on the first day of employment
- Subject to criminal background check and motor vehicle reports check
- Subject to pre-employment screening, including criminal background, drug testing, and/or education verification
