About the role

Key responsibilities & impact
  • Complete assessments for patients admitted to the hospital
  • Perform discharge planning and aftercare coordination, community referrals, and linkages
  • Provide patient and family education, adjustment to illness counseling, grief counseling, and crisis intervention
  • Promote patient- and family-centered care and document patient choices in the medical record
  • Recommend care and services appropriate to patients’ chronological age, developmental age, social maturity, and cultural background
  • Document and communicate with teams, physicians, and patient families regarding coordination of care and services
  • Serve as a consultant for guardianship, power of attorney, health care proxy, and advance directives
  • Collaborate with UAMS departments and participate in hospital-wide performance improvement and regulatory compliance initiatives
  • Report and review department-specific data metrics monthly
  • Assist the public and internal and external stakeholders with professional, friendly, and exceptional customer service
  • Maintain confidentiality and positive working relationships
  • Perform other duties as assigned to meet UAMS Core Values and department business needs

Requirements

What you’ll need
  • Masters' degree in Social Work
  • Must be licensed by an Arkansas Professional Board as a Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW) by date of hire
  • Certified Case Manager (CCM) or American Case Management (ACM) certification preferred
  • 2 years experience in an Acute Care Hospital Setting preferred
  • Experience in Case Management preferred
  • Proof of legal authority to work in the United States on the first day of employment
  • License or Certificate, List of three Professional References, and Resume required for application
  • Subject to pre-employment screening, including criminal background, drug testing, and/or education verification
