About the role

Key responsibilities & impact
  • Advance population health and value-based care initiatives across the Primary Care Population Health Service Line
  • Drive continuous quality improvement, operational success, and high-impact care delivery across multiple clinics and programs in Arkansas
  • Support Northwest Regional clinics in Fort Smith, Fayetteville, and Springdale to drive local and regional improvements in population health
  • Collaborate with access, nursing, physician, and administrative clinic leadership teams to align efforts and achieve organizational quality and performance metrics
  • Design and implement projects to improve care delivery, patient outcomes, and patient-centered care standards
  • Lead and mentor Population Health Improvement Assistants, if applicable
  • Work across clinics and programs to improve quality, safety, and patient experience outcomes
  • Report to the Quality Director for Population Health and Value-Based Care
  • Provide regular updates, align initiatives with organizational goals, and contribute to strategic population health planning

Requirements

What you’ll need
  • Bachelor’s degree in Health Information Management, Health Education, Public Health, Nursing, or a healthcare-related field
  • At least three (3) years in quality improvement, program management, or project management
  • Exceptional written and verbal communication to engage and influence diverse stakeholders
  • Proven expertise in performance/quality improvement, change management, and navigating regulatory compliance
  • Experience with Value-Based Care and Quality Improvement: HEDIS Measure, Risk Adjustment (HCC), and process improvement
  • Experience and knowledge with Electronic Health Records (EHR); EPIC experience preferred
  • Proficiency in Microsoft Office (Excel, Word, PowerPoint)
  • Experience using electronic medical record systems in clinical settings
  • Detail-oriented approach and ability to work independently while contributing effectively to a team
  • Strong knowledge of clinical quality measures and evaluation methodologies
  • Ability to travel frequently between clinical sites in Fort Smith, Fayetteville, and Springdale
  • Proof of legal authority to work in the United States on the first day of employment
  • Annual TB Screening, Criminal Background Check, Substance Abuse Testing
